Name, Father's Name, POR Card Number, Family ID, Province of Residence, City of Residence, Complete address, Nationality, Address

b. Educational Background
Category, Name of University, Program of Study, Degree Level, Degree Title, Major Subject, Duration of Degree, Total Semester, Current Semester.

c. Contact Details
Email Address, Primary Contact Number, Secondary Contact Number, Alternative Contact Number, Name of Alternative Contact Person, Relationship with Alternative Contact Person, social media. Details

d. DAFI Plus
Component Applied For, Round Participated Before, Component In which you participated before, what are you doing now a days, Courses of Preference, Reason to sign up for DAFI Plus, Consent for taking picture and Inclusion in database.
Duration:
The personal data will be stored with Inspire Pakistan for a period of 8 years or until the beneficiary requests to modify or delete the data.
Third-Party Access:
Your personal data will not be shared with any third parties unless required by law or with your explicit consent.
Rights:

• DAFI Plus information, including component applied for, round participated in before, courses of preference, reason for signing up, and consent for taking pictures and inclusion in the database

• I agree that I have read and understood the attached information sheet “We protect your data” on my data protection rights.
